Make a Referral

Pathway for a person with Aphasia

Gradual progression through stages supported through regular monitoring.

Stage 1

1:1 computer therapy focus
  • Focus on language outcomes.
  • Reviewed through formal SLT assessment with person with aphasia after period of six months (and end of iPad/app loan).
  • Decision to continue with stage 1 or to progress to stage 2 or 3.
  • Clients spend a maximum of 12 months at stage 1.

Stage 2

1:1 supported conversation focus
  • Focus on wellbeing/quality of life outcomes.
  • With or without iPad/computer therapy apps (client preference).
  • Monitored/reviewed by SWIT in discussion with volunteer, carer and client not through formal SLT assessment in most cases.
  • This will become the long term one to one support offer for people with aphasia.

Stage 3

Signposting to group support options
  • SWIT aphasia cafe.
  • SWIT carer support option.
  • Another support provider.

Speak With IT can receive referrals from speech and language therapists (SLT) but we also accept self referrals from people with aphasia, carers and other healthcare professionals or charities.

If you are an SLT please complete our Referral Form which can be found in the links on this page. Please include as much information as possible relating to a person’s spoken output, reading, writing and language comprehension as this will ensure the support we provide is guided from the outset.
